Analisis Kelengkapan Pengisian Formulir General Concent Rawat Jalan terhadap Standar Penilaian Akreditasi SNARS pada Elemen Penilaian HPK 5 di RSUD dr. Soedirman Kebumen

Formulir General Consent Rawat Jalan merupakan formulir baru dalam akreditasi SNARS dan baru pertama kali diterapkan di RSUD Dr. Soedirman. Terdapat formulir yang tidak sesuai dengan HPK 5. Tujuan penelitian ini adalah menganalisis kesesuaian pengisian formulir General Consent di RSUD Dr. Soedirman. Sampel yang digunakan yaitu Formulir General Consent Rawat Jalan 91 berkas. Metode penelitian yang digunakan adalah deskriptif kuantitatif. Hasil penelitian terdapat 5 indikator yang sudah terpenuhi lengkap, 6 indikator yang masih terpenuhi sebagian, dan 0 indikator yang tidak terpenuhi. Hasil Indikator Kesesuaian Indentitas Pasien adalah Nomor Rekam Medis 64,8%, Nama 69,2%, Tanggal Lahir 69,2%. Hasil Indikator Kesesuaian Indentitas Pemberi Persetujuan adalah Nama 95,6%, Tanggal Lahir 94,5%, Alamat 87,9%, Nomor Telepon 65,9%, Hubungan Keluarga 20,9%. Hasil Indikator Autentifikasi adalah Tanggal Persetujuan 80,2%, Nama dan tanda tangan Petugas 58,2%, Nama dan tanda tangan pemberi Persetujuan 83,5%. Kesimpulan pengisian formulir General Consent sudah sesuai dengan standar akreditasi elemen penilaian HPK 5.
